Disaster Relief Request
PROJECT: VetRelief provides disaster relief assistance to eligible active-duty military personnel, veterans, and their families residing in Florida who have been impacted by a recent natural disaster. This abbreviated application is intended specifically for hurricane and disaster relief efforts.
Required Documentation:
Please submit the following documents with your completed application. A PROJECT: VetRelief Case Worker may contact you for additional information to complete the review process.
- Proof of Military Service: DD-214 (Member 4 copy) showing an Honorable or Under Honorable Conditions discharge, or applicable active-duty documentation.
- Proof of Florida Residency: Valid driver’s license or other documentation confirming residency.
- Proof of Hurricane-Related Hardship: Photos of damage to your home or property and/or receipts documenting expenses resulting from displacement.
Type of Assistance:
Approved assistance will be provided in the form of food gift cards to help affected veterans and their families meet immediate needs. Assistance is subject to eligibility, review, and available funding.
